  • How do I request a FollowMyHealth patient portal account so I can view my health information online?

    There are two ways to request a portal account:

    • Print and follow the instructions on the Portal Access Request form.

    • Visit the Froedtert South FollowMyHealth login page and select " I need to sign up."

      • Follow these instructions to Sign up and request a portal connection.
      • It is necessary to bring your confirmation email and a valid ID to Froedtert South’s Health Information Management Department @ 6308-8th Ave for validation. Hours of operation are Monday through Friday 7:00am to 5:00pm.

    How do I access my FollowMyHealth patient portal account after I have registered for my account?

    Once the registration is complete, you can easily go directly to your health record using any computer or mobile app at any time.  We suggest you bookmark Froedtert South FollowMyHealth patient portal site:  https://uhsi.followmyhealth.com/ 

    What is Froedtert South FollowMyHealth Patient Portal?

    The FollowMyHealth patient portal is a secure, online web-based pathway that allows you, as a patient, to view and manage your personal health information.   

    What can FollowMyHealth do for me?

    FollowMyHealth provides you the opportunity to view online, download, and transmit your personal health information at your convenience. Currently, Froedtert South and Froedtert South Medical Group patients may view, track, and update their personal health information including:

    • Health Conditions
    • Allergies
    • Medications
    • View most lab results
    • Receive email and text messages when an update is made to your medical record
    • Pay Froedtert South bills online
    • View your physician’s profile
    • Set up proxy account for children under 12 and dependent adults
    • Access your portal account via mobile devices
    • Froedtert South Medical Group patients may request medication refills online

    Is my health information secure?

    FollowMyHealth was designed to give you access to your health care information without putting your privacy at risk. There are key features in place to be sure that your personal information can only be viewed by you.  You will be required to verify your identity during the online sign-up and login process.  Your private health information is protected and maintained securely by Froedtert South and the Allscripts FollowMyHealth team.  There are a number of security measures and standards in place to ensure that your data is protected when using the FollowMyHealth patient portal. 

    What will you use my email address for?

    • Your email address collected at registration will allow you to receive an auto-generated mail invitation from our FollowMyHealth patient portal. This email gives you step-by-step instructions on how to access your personal medical record online.
    • Your email address will be the Username for your account (only for accounts created after 5/11/2016)
    • Your email address will allow you to receive notifications when updates are sent to your portal account
    • Your email address will not be sold or be used for marketing purposes

    Can I use FollowMyHealth if I am not an existing Froedtert South patient?

    No, FollowMyHealth is for existing Froedtert South patients.  Once you have given your email, you will receive an email invitation to register for your FollowMyHealth portal account.

    I did not receive an email invitation to join Froedtert South FollowMyHealth patient portal?

    If you gave your email address to the Froedtert South registration staff, you should receive a FollowMyHealth email portal invitation within a few days.  If you did not receive an email, be sure to check your junk mail or quarantine file.

    Why do I need to provide my social security number when creating an account?

    We will need the last four digits of your Social Security number to create a personal access code for you to verify and activate your invitation. Your last four digits of your Social Security number is unique to you and is used as a secure identifier when you create your account.

    What if I forgot my user ID or password?

    • For accounts created after 5/11/2016, your Username is most likely your email address
    • For accounts created prior to 5/11/2016, look for the email notification containing your Username when you created your account or contact our support line at 262-656-3344
    • A password reset may be requested from the portal sign in page

    Will I be able to view my lab test results on the FollowMyHealth patient portal?

    The FollowMyHealth patient portal allows you to view most lab results performed at Froedtert South. It may take several days or weeks to view the lab result depending on the test ordered. Please contact your physician’s office if you have questions regarding your lab test.

    Will I be able to view my x-ray or other diagnostic test results on the FollowMyHealth patient portal?

    X-ray results and other diagnostic tests like EKG’s are not currently available on the patient portal. Please contact your physician’s office for your test results. Most lab results are available on the portal within 7 to 10 days.

    Can I have a FollowMyHealth patient portal account for my child?

    You may open a proxy account for your child or children, if they are under the age of 12.  You will need to complete a proxy verification form and present to Froedtert South’s Health Information Management Department @ 6308-8th Ave with a valid photo ID.  Hours of operation are Monday through Friday 7:00am to 5:00pm.

    Can I have a FollowMyHealth patient portal account for a dependent adult?

    If you are a legal guardian, have healthcare power of attorney or are responsible for the health decisions for a dependent adult, you may open a proxy account for the dependent adult.  You will need to complete a proxy verification form and present to Froedtert South’s Health Information Management Department @ 6308-8th Ave with a valid photo ID. Hours of operation are Monday through Friday 7:00am to 5:00pm.

    How do I know what my proxy account Invitation Code is?

    When creating a proxy account you will be asked for an Invitation Code. The invitation code is the four digit birth year (YYYY) of the oldest child or oldest dependent adult listed on the proxy email invitation.

    What if I have questions about the information contained in my personal health record?

    • Please contact your health care provider if you have a medical question.
    • Please contact Froedtert South Health Information Management Department at 262-656-2283, if you have questions about information contained in your personal health record.
    • To request a change to your medical record, please present to Froedtert South Health Information Management Department @ 6308-8th Ave with a completed health information change request form. Hours of operation are Monday through Friday 7:00am to 5:00pm.
